- The Cockcroft-Gault equation, developed in 1976, remains the most widely used basis for FDA drug-dosing labeling.
- Not accurate in AKI where renal function is changing rapidly; assumes a steady-state SCr.
- In obesity (BMI ≥30 or TBW > 130% IBW), using
ABWinstead of actual weight is recommended. - In underweight patients (TBW < IBW), use TBW as-is (do not substitute IBW).
- General dose-adjustment thresholds: CrCl <50 (adjust), <30 (reduce significantly), <15 (mostly contraindicated or dialysis-dependent).
- This result is a clinical decision-support aid; final prescribing decisions must account for the patient’s complete clinical picture.
