📌 Notes on the Calvert Formula
- Formula: Carboplatin dose (mg) = Target AUC × (GFR + 25) — the constant 25 accounts for non-renal clearance.
- GFR estimation: Cockcroft-Gault CrCl is the clinical standard; a measured GFR (e.g. ⁵¹Cr-EDTA) takes priority when available. CKD-EPI/MDRD tend to estimate lower than C-G and are not recommended for this purpose.
- Weight: In obese patients (BMI > 30), institutional protocol may call for IBW or Adjusted BW — be alert to overdosing risk.
- 125 mL/min GFR cap: The FDA recommends capping GFR/CrCl at 125 mL/min, based on 2010 overdose reports.
- Reduced renal function (CrCl < 60 mL/min): Increased risk of myelosuppression (e.g., thrombocytopenia) — consider a lower AUC target and G-CSF support.
- Minimum SCr: Some institutions cap the SCr used at no less than 0.7 mg/dL in women or sarcopenic patients.
