- This conversion is based on glucocorticoid (anti-inflammatory) potency. Mineralocorticoid effects must be considered separately.
- When converting after prolonged high-dose use, taper gradually (risk of HPA-axis suppression).
- Oral-to-IV conversion is generally 1:1, but since prednisone has no IV formulation, prednisolone is substituted.
- Dexamethasone has essentially no mineralocorticoid activity, so minimal effect on fluid/electrolytes.
- Hydrocortisone and cortisone have mineralocorticoid activity β watch for sodium retention/potassium loss with chronic use.
